Write down the decisions agents keep getting wrong
Record each design decision that is not obvious from the code, so agents stop undoing it.
How to do it
Keep short decision notes in the repository, such as docs/decisions/0007-no-orm.md, with the decision, the reason and the date. Link the folder from the instructions file. When an agent reverses a decision, add a check that fails if it happens again. See also keep decision records with a status.
How to check
The instructions file links the decisions folder, and every decision note has a decision, a reason and a date. Each decision an agent has reversed before has a check.
