# Write down the decisions agents keep getting wrong

- Rule: Record each design decision that is not obvious from the code, so agents stop undoing it.
- How: Keep short decision notes in the repository, such as `docs/decisions/0007-no-orm.md`, with the decision, the reason and the date. Link the folder from the instructions file. When an agent reverses a decision, add a check that fails if it happens again. - Check: The instructions file links the decisions folder, and every decision note has a decision, a reason and a date. Each decision an agent has reversed before has a check.
